LICOA gives you the freedom to use ANY provider. There is no special network.

Enjoy next-day coverage and no waiting periods on Type 1 and Type 2 procedures. Preventive visits, such as exams and cleanings, are covered up to 100%.

Dental

  • Can choose any dentist. There is no special network to worry about.

  • Exams, X-Rays, & cleanings covered at 100% on day one.

  • Basic benefits covered at 80% on day one to include extractions, fillings, full and partial denture repair, repair of crowns and bridges, and oral surgery.

  • Major benefits including endodontics, periodontics, crowns, bridges, dentures, inlays covered at 50% after one year (one year waiting period).

  • $50 annual deductible on Basic and Major services only

  • $1,200 maximum benefit on day one
