Vision (VSP Choice Network)

  • $10 co-pay for eye exam (every 12 months)

  • $20 co-pay for eyeglasses (every 12 months)

  • Frames allowance – up to $150 per year

  • PrimeStar Choice Monthly Rate: Employee - $16.34 *family rates available
